Dog Adoption Application

All of the information I have given is true and complete. This dog will reside in my home as a pet. I will provide proper and nutritious food, fresh water, indoor shelter, affection and kind treatment for the animal at all times. I agree to use only positive reinforcement and rewards-based training methods such as treats, praise, and toys. I promise to NEVER use aversive methods, pain, or fear based training or tools such as shock collars, prong collars, choke chains, leash corrections, or alpha rolls.  As with any dog, there is a chance that this dog could bite another dog or human and avoidance and training are highly recommended to ensure this doesn’t happen. This dog will receive annual physical examinations and regular vaccinations under the supervision of a licensed Veterinarian. I understand that the animal I am adopting has already been sterilized and microchipped and I agree that once I adopt this new dog and bring it home, any medical bills incurred on behalf of this animal or any other animals in my home will be my responsibility.

All dogs placed through the Wine Country Animal Lovers will have health checks, personality and temperament evaluations, and will be spayed or neutered prior to placement unless a medical condition exists, diagnosed by a veterinarian, that would prevent this surgery. I understand that this is a preliminary application and that there may be no Dog available in rescue at this time. I understand that if a Dog should become available, that I will be required to provide further information including, but not limited to, personal and veterinary references. I further understand that all information on my Preliminary Rescue Adoption Application will be kept confidential; that my application will be kept on file until I notify the Wine Country Animal Lovers that I am no longer interested in a rescue; and that I will not be contacted unless an appropriate Dog becomes available.
