Cat Adoption Application

All of the information I have given is true and complete. This cat will reside in my home as a pet. I will provide proper and nutritious food, fresh water, indoor shelter, affection and kind treatment for the animal at all times. I agree to use only positive reinforcement and rewards-based training methods such as treats, praise, and toys. I promise to NEVER use aversive methods, pain, or fear based training tools such as shock collars or spray bottles and I agree not to declaw this cat. This cat will receive annual physical examinations and regular vaccinations under the supervision of a licensed Veterinarian. I understand that the animal I am adopting has already been sterilized and microchipped and I agree that once I adopt this new cat and bring it home, any medical bills incurred on behalf of this animal or any other animals in my home will be my responsibility.

All cats placed through the Wine Country Animal Lovers will have health checks, personality and temperament evaluations, and will be spayed or neutered prior to placement unless a medical condition exists, diagnosed by a veterinarian, that would prevent this surgery. I understand that this is a preliminary application and that there may not be a cat that fits my preferences available in rescue at this time. I understand that if a cat should become available I may be asked to provide more information, but not limited to, personal and veterinary references. I further understand that all information on my preliminary Rescue Adoption Application will be kept confidential; that my application will be kept on file until I notify Wine Country Animal Lovers that I am no longer interested in adopting; and that I will not be contacted unless an appropriate cat becomes available.
